I like to go in depth on how I felt whilst reading it. I'll point out where I was tripped out of my flow by sentence structure or oddity. What confuses me. What intrigues me.
Examples can be seen in my recent reviews.
Happy to review privately on request. Default will be public.
Brutally honest, but fair. I will give my impressions of the main points: Plot, character, setting, dialog.
Then I will get into grammar, spelling, and typos.
I will tell you what works, and what doesn't.
As far as the star ratings go, I don't rely on them too much, but use them as a measure of an item's relative merit. Take it for what it's worth. Bottom line, my goal is to help the author improve their work.
I'll give a quick summary of your item. Afterwards, I'll talk about your characters, any Spelling or Grammar issues, and perhaps suggest some things, like adding the item to a Genre that it would fit into.
I have a form I use for reviews. I look for a Goal, Motivation, Conflict and Resolution. It also allows me to tell what I liked and what I thinks needs improving. In my reviews I may make suggestions by adding my take on what the author is trying to say. Purely a suggestion to take or leave as needed.
I try to find the WOW-factor in your writing. No in-depth analysis, just my thoughts.

I tend to focus on the technical aspects of a piece of writing, looking at the language, flow, style and punctuation.
I tend not to spend too much time on complimenting what works well.
I only send a review if I know I have something valuable to say. I tend to provide detailed advice, probably a longer and more detailed spiel than you actually wanted to be honest. I will sooner point out what you've done right then what you've done wrong, but I'm not afraid to be blunt when a piece needs serious revision. I will never criticize anything without offering advice on how to improve it - though I would rather see you ignore my advice altogether than make a change you don't want to make. It is your writing, after all.
Disclaimer: I am not good at remembering to do requested reviews. Often I have busy things going on with work and my own novels, so sorry if I decline or don't remember to do a review in the short time given. Just a warning.
When I review: Long. Depending on the type of story and reason for review I tend to get anywhere from 4,000 characters to on the rare occasion over 10,000.
I will make overall comments, technical points and even offer sources when necessary but a lot is dependent on what I'm reviewing. I can even do a full edit but that's take a lot of niceness and time.
Above all things, I believe reviews should be honest. I think that one brutally honest review can do more good than 100 coddling, everything-is-perfect reviews. I will never be mean (it's unprofessional) but if you're really serious about making your work better, I'll be happy to help.
My goal is to give a detailed, organized review that highlights the positive characteristics of the work, while also touching on any areas that might benefit from some editing. All of my reviews are, of course, just suggestions and meant to be helpful to the author. I am always available via WDC email if more input is needed on any aspect of the review.
I will be honest at all times - if there's something wrong with your piece, please don't be offended when I point it out. However, I will also give useful praise and constructive tips to help your word strike a chord!
Casual, friendly. I'll point out typos when I see them and make suggestions about quality, characters and logic, besides telling you how I feel about the piece.
I will write a detailed review of your item that goes into detail about the positives and negatives of your item. I color code each section so everything is easy to see once you view your review.
While I consider myself apt enough to provide decent, kind feedback, I'm not the sort of person to laud a man's writings. Instead, I seek to find smaller, more intricate points of interest within their writings so that I might offer suggestions and feedback so that they might better aid the flow of their work.
